PSM Request a Quote comes with default form fields and additional field types.
You can enable, disable, or customize these fields from Request a Quote → Form Settings.

Default Form Fields

These fields are available by default and cover basic customer information.

  • Billing Address
    Collects the customer’s billing details.

  • Shipping Address
    Collects the shipping address, if different from billing.

  • Message
    Allows customers to add notes or special requirements.

  • Attachment
    Lets customers upload files (for example, specifications or reference documents).

Additional Available Form Fields

You can add more fields by clicking Add New Field.

Text-based Fields

  • Text – Single-line input for short text.

  • Textarea – Multi-line input for longer messages.

Selection Fields

  • Checkbox – Allows selecting one or more options.

  • Single Select – Dropdown with one selectable option.

  • Multi-Select – Dropdown with multiple selectable options.

  • Radio – Choose one option from a list.

File & Date Fields

  • File – Upload a file.

  • Date – Select a date.

  • Time – Select a time.

Consent & Layout Fields

  • Acceptance – Checkbox for terms and conditions.

  • Heading – Adds a heading inside the form.

  • Divider – Adds a visual separator between fields.

Notes

  • You can activate or deactivate any field.

  • Fields can be marked as required if needed.

  • Use layout fields like Heading and Divider to improve form clarity.

Label

This is the name of the field shown to customers.
Example: Message, Company Name.

Extra Info

Adds a short description below the field.
Use it to guide users on what to enter.

Placeholder

Shows sample text inside the field.
The text disappears when the user starts typing.

Default Value

Pre-fills the field when the form loads.
Useful for fixed or suggested values.

CSS Class

Add custom CSS classes for styling.
Use commas to add multiple classes.

Required

Makes the field mandatory.
The form cannot be submitted if this field is empty.

Activate

Controls whether the field is visible in the form.
Turn it off to hide the field without deleting it.

Validation Settings (If Applicable)

Min Character Length

Sets the minimum number of characters required.
Helpful for fields like message or name.

Max Character Length

Limits the maximum number of characters allowed.
Prevents very long inputs.

Validation (Regex)

Validates the field value using a regular expression.
Useful for emails, phone numbers, or custom formats.
You can use tools like ChatGPT or Gemini to generate regex patterns.

Notes

  • Not all options are available for every field type.

  • Settings appear based on the selected field type.

  • Always save changes after updating a field.
